Before we can proceed with your clinical negligence case you must have made an official complaint to the NHS or the private hospital where you were treated.

This will help establish what happened and may provide much-needed answers to any questions you have.

If you have not yet done so please visit the NHS Inform website where you will find details on how to raise a complaint in your area. If your treatment was provided privately you should be able to find where to make the complaint via the website of the private hospital.

This complaint form should only take around five minutes to complete and will help us fully assess your case.
